Asthma Action Plan (English) / Asthma Action Plan (Spanish)
If your child has asthma and requires treatment and/or monitoring at school, an Asthma Action Plan must be completed by your child’s health care provider. This Asthma Action Plan will assist us in providing the best possible care for your child at school. Your child’s health care provider will be able to share pertinent information with the clinic staff to better manage your child’s asthma symptoms. After the health care provider completes it, parent/guardian should sign the bottom portion, and return to the school clinic. Please bring your child’s current medication with the Asthma Action Plan. A new action plan must be completed each school year.
