PISD Drug Testing Consent Form Required for Extracurricular Activities

Packets will be checked at the door. PISD medical packet must be completely filled out and the
Medical History section must be signed before a student is allowed enter.

The Packets of Pleasanton ISD athletes will be picked up at the end.

If you choose to go to your own Physician for a physical,
the completed packet will be due May 27, 2010 and can be turned in to the Athletic Office.
